- Why 4/10
- WINREVAIR is already FDA-approved (March 2024, updated October 2025) and EMA-approved (August 2024), with an expanded EU label decision by the European Commission expected in Q1 2026 — likely already concluded given we are mid-2026. No new PDUFA or pivotal Phase 3 readout is pending in the next ~12 months. The stock-moving binary (initial approval) has already passed. As an approved commercial-stage asset at a $325B diversified pharma, impactScore is capped at 4.
WINREVAIR is Merck's most important pipeline-to-launch transition since Keytruda — watch quarterly sales growth trajectory ($525M in Q1 2026, up from $280M in Q1 2025) and label expansion progress in HFpEF (Phase 2 CADENCE positive, Phase 3 planning underway). For a $325B company with Keytruda facing a 2028 patent cliff, WINREVAIR's ramp is a key swing factor in the 2028+ thesis, but MRK is far from a single-drug stock.

Executive Summary
WINREVAIR (sotatercept) is a first-in-class activin signaling inhibitor approved by the FDA in March 2024 and the EMA in August 2024 for pulmonary arterial hypertension (PAH). It generated $525M in Q1 2026 sales and is on a rapid growth trajectory toward analyst peak sales estimates of $3-7.2B by 2035. Three Phase 3 trials (STELLAR, ZENITH, HYPERION) all met primary endpoints with robust risk reductions of 76%. A Phase 2 proof-of-concept in HFpEF (CADENCE) was positive, opening a much larger addressable market. However, for Merck — a $325B market cap giant with $65B+ in annual revenue — WINREVAIR is an important growth driver but not a single-stock-defining binary. The stock price already reflects the approval; the thesis now hinges on commercial execution and label expansion success.
How It Works
WINREVAIR is an engineered protein that blocks a signaling pathway called activin, which is overactive in pulmonary arterial hypertension (PAH). By inhibiting this signal, it helps relax and remodel the blood vessels in the lungs, making it easier for the heart to pump blood through them. It's the first drug in its class for PAH — instead of just dilating vessels like older drugs, it targets the underlying disease process.
Trial Status
FDA approved March 2024 based on Phase 3 STELLAR trial. Phase 3 ZENITH (high-risk PAH, March 2025) showed 76% reduction in morbidity/mortality. Phase 3 HYPERION (newly diagnosed, September 2025) showed 76% reduction in clinical worsening. Phase 2 CADENCE (CpcPH-HFpEF, November 2025) met primary endpoint — full results presented March 2026. Approved in over 50 countries.
The Two-Sided Case
