- Time to Market
- Already approved (Dec 2023 in US; March 2025 in EU). ATTR-CM indication now likely delayed 3-5+ years if pursued at all.
- Why 4/10
- Wainua is already approved and commercialized for hATTR-PN, so its defining binary catalyst has passed. The next hard catalyst is the full CARDIO-TTRansform data presentation at the European Society of Cardiology Congress in August 2026, but since the top-line primary endpoint already clearly failed, this event is unlikely to be a positive swing factor. For AstraZeneca ($587B market cap), Wainua is a minor asset relative to the overall portfolio, capping the impact score at 4.
Wainua's CARDIO-TTRansform Phase 3 failure in ATTR-CM (July 9, 2026) effectively closes off the much larger cardiomyopathy opportunity. Focus on the August 2026 ESC presentation for whether the monotherapy subgroup signal is strong enough to justify a second trial. For AZN, this is a manageable pipeline setback; IONS shareholders face a more meaningful valuation reset since ATTR-CM was a key growth driver for that stock.

Executive Summary
Wainua (eplontersen) is an FDA-approved antisense oligonucleotide for hereditary transthyretin amyloidosis with polyneuropathy (hATTR-PN), co-developed and commercialized by AstraZeneca and Ionis Pharmaceuticals. It generated $212M in global sales in 2025. However, on July 9, 2026, the pivotal CARDIO-TTRansform Phase 3 trial in the much larger ATTR cardiomyopathy (ATTR-CM) indication failed its primary endpoint of reducing cardiovascular mortality and recurrent CV events. This is a significant setback because ATTR-CM represents a blockbuster market already dominated by Pfizer's tafamidis (~$4B) and contested by BridgeBio's acoramidis and Alnylam's vutrisiran. For AstraZeneca ($587B cap), the impact is manageable; for Ionis (~$5B cap), it is a more severe blow to its growth narrative.
How It Works
Eplontersen is a synthetic piece of genetic material (an antisense oligonucleotide) designed to block the production of transthyretin (TTR), a protein that can misfold and form toxic clumps (amyloid deposits) that damage nerves and the heart. It is delivered as a once-monthly injection that patients can self-administer. The "LICA" technology helps the drug reach the liver (where TTR is made) more efficiently.
Trial Status
Approved for hATTR-PN (polyneuropathy indication) based on positive Phase 3 NEURO-TTRansform trial. The Phase 3 CARDIO-TTRansform trial for the larger ATTR-CM (cardiomyopathy) indication failed its primary endpoint on July 9, 2026. A smaller Phase 3 EPIC-ATTR trial in Chinese patients with ATTR-CM (N=64) is active but not recruiting, with estimated primary completion May 2026 and final completion January 2027. Full CARDIO-TTRansform data to be presented at ESC Congress August 2026.
The Two-Sided Case
Upcoming Catalysts
- Full CARDIO-TTRansform Phase 3 data presentation at the European Society of Cardiology (ESC) Congress, August 2026.
- Regulatory strategy decisions on ATTR-CM — whether to pursue a narrower indication or design a new monotherapy trial.
- EPIC-ATTR Phase 3 Chinese ATTR-CM trial final completion (estimated Jan 2027).
- Quarterly sales trajectory for Wainua in hATTR-PN (next earnings: AZN H1 2026 results expected late July 2026).
