- Trial Phase
- Phase 2b completed; Phase 2a (VIRAGE2) authorized/enrolling; Phase 3 design aligned with FDA and EMA
- Time to Market
- Likely 4-6 years if Phase 3 is successful, assuming the company secures funding/partnership to initiate a pivotal trial
- Why 5/10
- No single hard dated PDUFA or pivotal readout is scheduled within the next ~12 months; the VIRAGE2 Phase 2a trial (6 patients, authorized July 2026) is a small dosing-regimen feasibility study, not a pivotal binary. The Phase 3 has been designed with FDA/EMA alignment but is unfunded. ImpactScore is 5 because VCN-01 is Theriva's lead (near sole) asset with positive Phase 2b data, but the next binary catalyst is too distant to score higher.
VCN-01 is the only meaningful asset at a ~$10M market-cap company. The positive VIRAGE Phase 2b data and FDA/EMA alignment on Phase 3 design are real positives, but the company has only ~$14M cash (runway into Q1 2027) and needs a partnership or dilutive financing to fund the pivotal trial. Watch for partnership/licensing announcements and VIRAGE2 interim data as the next catalysts — the stock is highly binary and prone to large swings.

Executive Summary
VCN-01 is an oncolytic adenovirus (zabilugene almadenorepvec) being developed by Theriva Biologics (TOVX, ~$10M market cap) primarily for metastatic pancreatic ductal adenocarcinoma (PDAC). A 112-patient Phase 2b trial (VIRAGE) met its primary endpoints in 2025, showing a meaningful survival benefit (median OS 10.8 vs 8.6 months, HR ~0.44 for death risk reduction) when combined with standard chemo. The FDA and EMA both aligned on a Phase 3 design in early 2026. However, the company has only ~$14M cash (runway into Q1 2027) and cut 32% of its workforce to conserve capital. The pivotal Phase 3 is unfunded, making partnership or dilutive financing a critical near-term need. A major competitive overhang is Revolution Medicines' daraxonrasib, which nearly doubled survival in a 500-patient Phase 3 trial for KRAS-mutant PDAC (~90% of cases). VCN-01 targets a different mechanism (tumor stroma, all-comers regardless of mutation) and could be complementary, but daraxonrasib's data has reshaped the pancreatic cancer treatment landscape.
How It Works
VCN-01 is an engineered cold virus (adenovirus) designed to selectively infect and replicate inside cancer cells. It has a built-in enzyme (hyaluronidase) that breaks down the dense, protective scar tissue surrounding pancreatic tumors — essentially cutting through the tumor's "armor" so chemotherapy and immune cells can reach the cancer. It also turns immunologically "cold" tumors into "hot" ones that the immune system can recognize and attack.
Trial Status
The VIRAGE Phase 2b trial (112 patients, randomized) in first-line metastatic pancreatic ductal adenocarcinoma (PDAC) met its primary efficacy and safety endpoints in May 2025. Results presented at ESMO 2025 and AACR 2026 showed VCN-01 + chemo improved median OS to 10.8 months vs 8.6 months for chemo alone, with a 56% reduction in death risk. Two doses separated by 3 months outperformed one dose. A smaller VIRAGE2 Phase 2a trial (6 patients, single-arm) was authorized by Spanish regulators in July 2026 to test more frequent (≥3) dosing to inform the Phase 3 regimen. Phase 3 protocol is finalized with FDA/EMA input but not yet initiated.
The Two-Sided Case
Upcoming Catalysts
- VIRAGE2 Phase 2a (6-patient) trial enrollment updates and interim safety/tolerability data — authorized July 2026 by Spanish AEMPS
- Phase 3 trial initiation announcement — company is pursuing strategic partnership or funding opportunities to support the pivotal study
- Potential partnership or licensing deal for VCN-01 in PDAC or retinoblastoma
- Retinoblastoma Phase 1 data presentations at medical conferences (e.g., ASCO, APAO)
- Quarterly earnings updates providing cash position and business development updates
