- Already approved (FDA approval June 17, 2026). GSK is now commercializing the drug.
- Why 4/10
- Utebzi was approved by the FDA on June 17, 2026. As an already-approved, marketed drug with no pending regulatory decision or pivotal readout in the next ~12 months, there is no binary catalyst to drive a large share-price move. The remaining pipeline (SPR720, SPR206) is weak — SPR720 Phase 2a missed its primary endpoint and SPR206 was halted. ImpactScore is 4: approved drug, no hard dates ahead.
The approval binary has passed; the stock now trades on Utebzi commercial uptake (milestones/royalties from GSK) rather than catalyst-driven moves. Watch for GSK's commercial launch updates and any milestone-triggering events. The rest of Spero's pipeline is weak (SPR720 Phase 2a failed, SPR206 halted), making this a pure-play royalty/milestone story off a single approved drug.

Executive Summary
Tebipenem pivoxil hydrobromide (Utebzi) is the first FDA-approved oral carbapenem antibiotic, approved June 17, 2026 for complicated UTIs in adults. Developed by Spero Therapeutics (Nasdaq: SPRO) and licensed to GSK in 2022 for $66M upfront (up to $525M in milestones + tiered royalties), after the FDA had initially issued a Complete Response Letter in 2022 requiring an additional Phase 3 trial. The stock surged ~245% when the PIVOT-PO Phase 3 trial hit in mid-2025 and rose another ~16% on approval, but has since settled. With the rest of Spero's pipeline struggling (SPR720 Phase 2a missed its endpoint, SPR206 halted), the company's value now hinges entirely on its GSK partnership and Utebzi's commercial trajectory — a less binary, more gradual story than the pre-approval days.
How It Works
Utebzi is an oral antibiotic (a carbapenem, the same class as meropenem and ertapenem, which are normally given by IV). It is a prodrug that converts to its active form (tebipenem) in the blood, where it kills a broad range of Gram-negative bacteria including drug-resistant strains. It is the first and only oral carbapenem approved in the US, allowing patients with complicated UTIs to avoid hospitalization for IV antibiotics.
Trial Status
FDA approved Utebzi on June 17, 2026 for complicated urinary tract infections (cUTI), including pyelonephritis. Approval was based on the Phase 3 PIVOT-PO trial (NCT06059846) which met its primary endpoint — non-inferiority of oral tebipenem HBr vs intravenous imipenem-cilastatin — and was stopped early for efficacy in June 2025. Full results were presented at IDWeek in October 2025.
The Two-Sided Case
Upcoming Catalysts
- GSK commercial launch of Utebzi in the US (post-June 2026 approval) — milestone payments to Spero upon first commercial sale and tiered royalty revenue thereafter.
- Potential milestone triggers from GSK under the $525M milestone pool (development, sales, and commercial milestones).
- Possible label expansion (e.g., pediatric indication) — though no specific trials or timelines have been announced.
- SPR720 and SPR206 pipeline updates — both programs have faced setbacks, diminishing the pipeline catalyst angle.
