- Trial Phase
- Phase 2b (registrational)
- Time to Market
- Potentially 2028-2029 if Phase 2b succeeds, given the registrational trial topline reads out in 2H 2027 and accelerated approval is possible via the C-peptide surrogate endpoint pathway
- Why 4/10
- The next hard binary catalyst is topline data from the registrational Phase 2b SAFEGUARD trial, expected in 2H 2027 — ~12-18 months out. Per the rubric, a mid-stage drug whose next catalyst is >12 months away caps at 5; SAB-142 is the company's only clinical asset, so it sits at a 4.
SAB-142 is early-stage but has strong institutional validation and ample cash runway through 2028. The next major catalyst (Phase 2b topline data) is still over a year away, so near-term moves will be driven by enrollment milestones, conference data presentations, and progress on the PRISE-hATG study. The stock carries elevated short interest (~56-61% short ratio), so positive enrollment updates could trigger short squeezes. For investors, this is a pre-revenue binary bet with no near-term FDA decision — sizing accordingly is key.

Executive Summary
SAB-142 is SAB Biotherapeutics' only clinical-stage asset — a fully human, re-dosable anti-thymocyte globulin (hATG) being developed as a disease-modifying therapy for autoimmune type 1 diabetes. Phase 1 data showed C-peptide preservation and a clean safety profile. The registrational Phase 2b SAFEGUARD trial is actively enrolling, with topline data expected in 2H 2027. The company has $217.6M in cash (runway through 2028), strong institutional backing (RA Capital, Balyasny, Adage Capital, Millennium, BlackRock all added in Q1 2026), and a consensus Strong Buy rating from 11 analysts with a mean price target of $11.18 (vs ~$3.92). The recent FDA accelerated approval of Sanofi's Tzield (teplizumab) for Stage 3 T1D in June 2026 partially de-risks both the regulatory pathway and the commercial thesis for SAB-142.
How It Works
SAB-142 is an antibody therapy that calms the overactive immune system in type 1 diabetes. It works by targeting and modulating multiple types of immune cells that mistakenly attack the insulin-producing beta cells in the pancreas. This is the same mechanism as rabbit-derived ATG (which has been shown in multiple trials to slow T1D progression), but SAB-142 is fully human — meaning it can be re-dosed safely over time without triggering an immune reaction against the drug itself. It's given as an IV infusion.
Trial Status
Phase 1 completed Dec 2025 — met primary endpoint, showed C-peptide preservation and improved glycemic control with a favorable safety profile in both healthy volunteers and established T1D patients. Phase 2b SAFEGUARD trial (NCT07187531) dosed its first patient in Dec 2025, targets ~159 patients aged 5-40 with new-onset Stage 3 T1D. Enrollment on track to complete by end of 2026. Breakthrough T1D (the leading T1D advocacy org) awarded a grant in July 2026 to support a separate PRISE-hATG study of SAB-142 in patients 100 days to 2 years post-diagnosis.
The Two-Sided Case
Upcoming Catalysts
- Enrollment completion for Phase 2b SAFEGUARD trial expected by end of 2026
- Phase 2b SAFEGUARD topline data expected in 2H 2027 (primary binary catalyst)
- Additional data presentations at ADA, EASD, and other medical conferences
- PRISE-hATG study (new Breakthrough T1D grant award) initiation and enrollment updates
- Potential FDA interactions regarding accelerated approval pathway updates
