- Why 4/10
- Niktimvo is already FDA-approved and generating revenue — the next hard catalysts are Phase 2 data readouts in IPF (MAXPIRe) and earlier-line cGVHD combo (both Q4 2026). These are not pivotal/registrational readouts, so the binary is muted. ImpactScore 4 reflects an approved drug with no pending regulatory decision.
Niktimvo's commercial trajectory is the primary value driver for SNDX on this drug. Watch Q2/Q3 2026 net revenue prints for signs of continued ramp ($151.6M FY2025 annualizing to ~$220M in Q1 2026). The Phase 2 IPF readout in Q4 2026 is an important optionality catalyst that could open a much larger market, but comes with clinical risk. The $250M convertible note issued June 2026 introduces modest dilution overhang (~8-10% of shares outstanding).

Executive Summary
Niktimvo (axatilimab-csfr) is an FDA-approved first-in-class CSF-1R-blocking antibody for chronic graft-versus-host disease (cGVHD) after ≥2 prior lines of therapy, co-developed and co-commercialized by Syndax Pharmaceuticals (SNDX) and Incyte (INCY). Approved in August 2024, it generated $151.6 million in net revenue to Incyte in its first full year (2025), with Syndax recording $42.4M in collaboration revenue (50% of net commercial profit). The drug already annualizes at roughly $220M as of Q1 2026. Beyond cGVHD, a Phase 2 IPF (idiopathic pulmonary fibrosis) trial (MAXPIRe) is reading out in Q4 2026 — a potential multi-billion-dollar market if positive. Analyst consensus on SNDX is Strong Buy (19 Buy, 1 Hold) with a mean price target of $39, implying ~76% upside from ~$22.
How It Works
Niktimvo is an antibody that blocks the CSF-1 receptor, a protein on the surface of certain immune cells (monocytes and macrophages). By blocking this receptor, it reduces the activity of these immune cells, which are overactive in chronic graft-versus-host disease and drive tissue inflammation and scarring. It is a first-in-class treatment that targets a different pathway than existing cGVHD therapies.
Trial Status
Niktimvo won FDA approval in August 2024 for chronic GVHD after ≥2 prior lines of therapy based on the Phase 2 AGAVE-201 trial (79 patients, 0.3 mg/kg Q2W). The trial showed a high overall response rate. Long-term follow-up data presented in late 2025 confirmed a tolerable safety profile and maintained responses. Additional vial sizes (9 mg and 22 mg) were approved January 2025. The MAXPIRe Phase 2 trial in IPF (idiopathic pulmonary fibrosis) is fully enrolled (145 patients) with topline data expected Q4 2026. A Phase 2 trial of axatilimab + ruxolitinib in earlier-line cGVHD is also reading out in Q4 2026.
The Two-Sided Case
Upcoming Catalysts
- Q4 2026: Phase 2 MAXPIRe topline data for axatilimab in IPF (idiopathic pulmonary fibrosis)
- Q4 2026: Phase 2 axatilimab + ruxolitinib data in earlier-line cGVHD
- Ongoing: Quarterly Niktimvo net revenue reports from Incyte (Syndax collaboration revenue lagged by ~1 quarter)
