- Why 4/10
- BRINSUPRI is already FDA-approved and commercially launched in the US, EU, and Japan with no pending PDUFA or pivotal readout for its core NCFB indication in the next ~12 months. Per the rubric, an approved marketed drug with no imminent new regulatory decision scores 3-5; this drug is the primary value driver for Insmed but lacks a hard binary catalyst ahead.
BRINSUPRI is already a commercial success with a blockbuster launch trajectory, but the stock has pulled back ~40% from its 52-week high of $212.75. With no imminent binary catalyst (the drug is already approved globally), the investment thesis hinges on continued commercial execution and pipeline progress from Insmed's other assets (TPIP, ARIKAYCE). Watch quarterly prescription trends and any label-expansion updates for brensocatib.

Executive Summary
BRINSUPRI (brensocatib) is the first and only FDA-approved treatment for non-cystic fibrosis bronchiectasis (NCFB), a chronic lung disease with no prior approved therapy. Approved by the FDA on August 12, 2025, and subsequently in the EU (November 2025) and Japan (August 2026), the drug is a first-in-class oral DPP1 inhibitor that targets neutrophil-driven airway inflammation. The commercial launch has been exceptionally strong — Q2 2026 BRINSUPRI revenue hit $309.2 million (49% sequential growth), and management raised full-year 2026 guidance to $1.25B-$1.40B. Insmed trades at ~$126 with a consensus analyst price target of ~$200 (56% upside), supported by 29 of 30 analysts rating Buy/Strong Buy. However, the drug failed Phase 2 trials in hidradenitis suppurativa and chronic rhinosinusitis, and insider selling has been notable.
How It Works
BRINSUPRI is an oral pill that blocks an enzyme called DPP1, which is needed to activate certain inflammatory proteins (neutrophil serine proteases) in white blood cells. By reducing the activity of these inflammatory proteins, the drug decreases airway inflammation and damage in bronchiectasis patients, leading to fewer lung infections (exacerbations).
Trial Status
FDA-approved on August 12, 2025 based on the Phase 3 ASPEN trial (N=1,719), which showed brensocatib significantly reduced pulmonary exacerbations vs placebo over 52 weeks and met multiple key secondary endpoints. The drug was also approved by the European Commission in November 2025 and by Japan's MHLW in August 2026. Phase 2b CEDAR trial in hidradenitis suppurativa failed to meet efficacy endpoints, and a Phase 2 trial in chronic rhinosinusitis without nasal polyps also failed.
