- Trial Phase
- Approved (Phase 3 completed for Alzheimer's agitation; Phase 2/3 planned for smoking cessation)
- Time to Market
- Already on the market — originally approved for MDD in August 2022; new Alzheimer's agitation indication launched June 2026.
- Why 4/10
- AXS-05 (Auvelity) is already FDA-approved for both major depressive disorder (Aug 2022) and Alzheimer's disease agitation (April 30, 2026). There is no hard, dated binary regulatory catalyst upcoming for this specific drug in the next ~12 months. The next catalyst is the commercial ramp of the newly approved Alzheimer's agitation indication (launched June 2026), plus a Phase 2/3 smoking cessation study starting Q2 2026 with readout likely >12 months away. As an approved, marketed drug, impactScore is capped at 4-5 per rubric.
Auvelity's recent FDA approval for Alzheimer's agitation (April 30, 2026) opens a large new addressable market. Watch Q2 2026 and Q3 2026 earnings for early commercial traction in this new indication — revenue growth trajectory will determine whether the stock can sustain its ~155% 52-week rally. The Phase 2/3 smoking cessation trial starting Q2 2026 is a pipeline upside catalyst but likely >12 months from a readout.

Executive Summary
AXS-05 (Auvelity) is Axsome Therapeutics' lead commercial drug — an oral, fixed-dose combination of dextromethorphan and bupropion. Originally FDA-approved for major depressive disorder in August 2022, it became the first oral NMDA receptor antagonist and first new mechanism of action for MDD in over six decades. On April 30, 2026, the FDA significantly expanded the label to include agitation associated with Alzheimer's disease dementia — the first non-antipsychotic ever approved for that indication. Auvelity generated $507.1 million in U.S. net product sales in FY2025 (74% YoY growth), and management projects peak annual sales of at least $8 billion across both indications. The commercial launch for Alzheimer's agitation began in June 2026. The stock has rallied strongly (~155% from its 52-week low) on the approval and raised guidance, but as an already-approved drug with no new binary regulatory catalyst pending near-term, the next 12 months are about commercial execution rather than a binary event.
How It Works
AXS-05 combines two older drugs — dextromethorphan (a cough suppressant) and bupropion (an antidepressant/stop-smoking aid). Dextromethorphan blocks the NMDA receptor in the brain (like the newer antidepressant Spravato, but as a pill instead of a nasal spray) and also activates the sigma-1 receptor. Bupropion serves two roles: it inhibits the liver enzyme CYP2D6, which dramatically raises dextromethorphan blood levels, and it provides its own antidepressant effect as an NDRI. The result is a fast-acting, oral antidepressant with a novel mechanism — the first new oral mechanism for major depression in over 60 years.
Trial Status
Auvelity is already FDA-approved and commercialized for two indications. The pivotal Phase 3 ACCORD-2 and ADVANCE-2 trials supported the Alzheimer's agitation sNDA approval. A Phase 2/3 trial for smoking cessation is expected to start in Q2 2026.
The Two-Sided Case
Upcoming Catalysts
- Q2 2026 earnings (late July/Aug 2026): first full quarter of Auvelity sales including initial Alzheimer's agitation launch contribution.
- Phase 2/3 smoking cessation trial initiation (expected Q2 2026).
- Commercial launch traction updates for Alzheimer's agitation indication across 2026.
- Q3 2026 earnings (late Oct/early Nov 2026): further indication of Alzheimer's agitation uptake and revenue growth trajectory.
