- Why 6/10
- The next hard dated catalyst is the FDA decision on AVT16's aBLA, which Alvotech expects in Q1 2027 — roughly 6-8 months from the current date. This is a binary regulatory decision on a major value driver (first vedolizumab biosimilar in FDA review, partnered with Teva, targeting a ~$6B reference market), but Alvotech also has two earlier FDA decisions pending (AVT05/AVT06, late 2026) and existing commercial revenue, so AVT16 is not a single-asset make-or-break. Hence score 6.
Watch for (1) any FDA review updates on AVT16 ahead of the expected Q1 2027 decision; (2) the earlier AVT05/AVT06 FDA decisions (late 2026) as a read-through on manufacturing compliance; (3) Fresenius PB016's review timeline, which determines how long Alvotech's first-mover window stays open; (4) further financing/dilution given the ~$107.6M Q1 2026 cash burn and high-cost debt.

Executive Summary
AVT16 is Alvotech's proposed interchangeable biosimilar to Takeda's Entyvio (vedolizumab), a blockbuster IV biologic for ulcerative colitis and Crohn's disease. Its aBLA was the first vedolizumab biosimilar accepted for FDA review (June 8, 2026), with a decision expected in Q1 2027. The reference product generated ~$6B in Takeda's FY2024/25, and AVT16 would be marketed by Teva in the US. A May 2026 FDA manufacturing inspection closed with a favorable VAI classification in July 2026, clearing a key regulatory overhang. However, Fresenius Kabi/Polpharma's competing vedolizumab biosimilar (PB016) had its BLA accepted just two months later (July 31, 2026), narrowing the first-mover window, and Alvotech's history of CRLs and recent dilutive equity raises add risk.
How It Works
Vedolizumab is a lab-made antibody that blocks a gut-specific 'homing' protein (alpha-4 beta-7 integrin) on white blood cells, preventing them from entering the bowel lining and causing inflammation. It is used as an IV infusion to treat moderate-to-severe ulcerative colitis and Crohn's disease. AVT16 is a near-copy (biosimilar) of that antibody, designed to work identically at a lower cost.
Trial Status
Alvotech completed a Phase 1 pilot PK/safety study in healthy volunteers (NCT06400719) and commenced a Phase 3 efficacy trial in moderate-to-severe ulcerative colitis in September 2024. The aBLA was accepted for FDA review on June 8, 2026, supported by analytical, PK, and immunogenicity data. A separate Phase 1 PK study for the subcutaneous formulation (AVT80) met all primary endpoints in February 2026.
