- Time to Market
- Approximately 3-5 years, assuming positive Phase 2b (data 2H 2027), Phase 3 trials starting in early 2027, and a potential NDA submission thereafter.
- Why 5/10
- The next hard, dated catalyst is Phase 2b topline data for ALTO-207 in TRD expected in 2H 2027 (~12-18 months out). ALTO-207 is the company's lead asset and primary value driver. However, with the next binary readout >12 months away and the stock already up ~940% over the past year, near-term share-price impact is capped at moderate until data reads out.
ALTO-207 is ANRO's lead value driver. Watch for Phase 2b enrollment completion updates and any early data glimpses. The Phase 2b topline readout (2H 2027) is the big binary. The strong $244M cash runway through 2029 (per Jul 2026 8-K) and FDA alignment reduce financing and regulatory risk, but the stock's ~10x run from its 52-week low means much optimism is already priced in. The Phase 2a data (Cohen's d=1.67 at week 6) was strong but came from only 32 patients.

Executive Summary
ALTO-207 is a fixed-dose combination of pramipexole (dopamine agonist) and ondansetron (anti-emetic) being developed by Alto Neuroscience (NYSE: ANRO) for treatment-resistant depression (TRD). It was acquired from Chase Therapeutics in June 2025 for $1.75M upfront plus milestone payments up to $71.5M. A Phase 2a trial in 32 MDD patients showed strong efficacy signals (Cohen's d=1.67 on MADRS at week 6) with good tolerability. A Phase 2b potentially pivotal trial (~178 patients) began in April 2026, with topline data expected 2H 2027. The company has ~$244M cash (through 2029), strong analyst support (16 buy/strong-buy, mean PT $35.13 vs ~$26 currently), and recently announced an expanded Phase 3 monotherapy program. The main risk: Phase 2b data is >12 months away, and the stock has already rallied ~940% from its 52-week low, pricing in significant optimism.
How It Works
ALTO-207 is a pill that combines two older, well-understood drugs: pramipexole (a dopamine receptor stimulator, currently approved for Parkinson's and restless legs) and ondansetron (an anti-nausea drug). Pramipexole boosts dopamine signaling in the brain's reward circuits, which can lift depression — especially anhedonia (loss of pleasure). But pramipexole alone causes nausea, limiting how fast patients can reach an effective dose. Adding ondansetron blocks that nausea, allowing patients to ramp up to a therapeutic dose much faster and with fewer side effects.
Trial Status
A Phase 2a trial (32 patients with MDD) met primary and secondary endpoints with statistically significant MADRS improvements (Cohen's d=1.67 at week 6, p=0.0004). A Phase 2b potentially pivotal trial in ~178 TRD patients was initiated in April 2026 with topline data expected in 2H 2027. An additional Phase 3 monotherapy trial was announced in July 2026, planned for early 2027.
